This form is used to send the Master Score Card to the League Secretary and Webmaster. You can send it as a photograph or PDF.

Please ensure

1.  The image is in portrait mode.

2.  The writing on the Master Card is clear and easy to read.

3.  That team names are included where specified on the Master Card

4.  That none of the Master Card was truncated when you were photographing it..

5.  Where a scheduled match is not played complete the Match Not Played Section.

This is the league for which you are submitting results. The leagues are in the order in which they are played. They start with the Saturday Morning League (The Tormey Cup and finish with the Thursday Afternoon League (The McIntyre Cup).
Date the match was originally scheduled to be played. Note : In the case if postponed or brought forward matches this will not be the date actually played.
